An automated patient triage engine that calculates clinical risk levels, triggers multi-channel notifications (Email/SMS), and manages the lifecycle of disposable diagnostic devices via API.

Not a fit for healthcare providers whose diagnostics must stay purely manual. Also not a fit where specific local regulatory constraints bar automated logic from prioritizing patient outreach.
The Challenge
Sleep assessments ran on a labor-intensive legacy process, and leads decayed inside it. A patient would complete an online assessment, then the data sat idle until a clinician manually reviewed the responses and assigned a risk level. The delay often passed 48 hours. By then, patient intent had cooled. Disposable Home Sleep Test (HST) devices made tracking worse. Standard CRM pipelines weren't built for the rapid "one-way" deployment of disposable hardware, where traditional "return" stages are irrelevant.
Our Approach
We rebuilt the intake logic to treat the CRM as a real-time triage engine. Clinical diagnostic criteria map to a series of hidden "Risk Score" properties within HubSpot. On form submission, workflows instantly calculate the risk level, High, Medium, or Low, and append it to the contact record. High-risk patients skip the standard nurture tracks. The system immediately triggers an SMS via SMSZap and creates a high-priority Task for the clinical coaching team.
For the HST device integration, a custom "Auto-Populate" workflow captures device serial numbers when users self-register. Manual data entry drops out, and the patient record updates before the first diagnostic night is complete.
